Description

Vemlidy is an antiviral medicine that works by stopping or slowing down the growth and spreading of either HIV-1 or HBV virus

Vemlidy consists of Tenofovir Alafenamide, a prodrug of Tenofovir Disoproxil. The active substance, known as a nucleotide reverse transcriptase inhibitor (NRTI), is used to treat a long-term infection caused by the hepatitis B virus in patients with stable (compensated) liver disease, which means that the liver is damaged but can still perform its main functions. The medicine manages the infection by suppressing the virus multiplication and spread, thereby preventing further liver cells contamination and reducing the virus amounts in the body.

Tenofovir Alafenamide present in Vemlidy has also shown its efficiency in fighting against the human immunodeficiency virus (HIV-1). Due to the risk of HIV drug resistance, the medication is most often utilized as a component of concomitant anti-HIV-1 therapy. Together with other antiviral medicines, it prevents HIV-1 from making copies of itself and spreading through the body which helps reduce the concentration of the virus and strengthen the immune system.

The medication does not stop the transmission of infectious diseases like HIV-1 or HBV which are passed through the blood or sexual contacts. Patients should, thereby, be advised to continue taking appropriate precautions.

Indications

Treatment of human immunodeficiency virus (HIV-1) infection concomitantly with other antiviral agents.

Treatment of chronic hepatitis B virus (HBV) infection in patients with compensated liver disease (liver is damaged but still works properly).

MOA (mechanism of action)

Tenofovir Alafenamide is a nucleotide reverse transcriptase inhibitor (NRTI) and a new ester prodrug of Tenofovir Disoproxil. Following the oral administration, the active substance is converted into its active metabolite called Tenofovir Diphosphate. It suppresses the action of viral reverse transcriptase (RT) by competing with natural deoxyribonucleotide substrates for incorporation into the infected DNA and stopping the binding processes required for DNA extension. This activity results in chain termination and prevention of viral DNA transcription.

Dosage

Prior to medicine’s initiation, patients should undergo blood tests to determine an appropriate dose for each individual. Vemlidy is meant for oral use only. It should be administered on a once-a-day dosing schedule with food. The maximum daily dose of Vemlidy is 25 mg.

The medicine intake must not be changed or discontinued without first consulting a doctor as this may cause severe acute disease exacerbations. Patients should be under constant medical supervision and take regular blood tests so that the treatment of either HIV-1 or HBV is closely monitored and, if needed, adjusted.

Missed dose

If you miss a dose of Vemlidy you should take it as soon as you remember. If it is the time for the next dose you should continue your regular dosing schedule. Do not take a double dose to make up for a missed one.

Overdose

If an overdose occurs and you are feeling unwell, you should seek emergency medical attention or contact your healthcare provider immediately.

Storage

Store at room temperature under 30°C (86°F) away from excess moisture and heat. Do not use Vemlidy if the seal is missing or broken. Keep the bottle tightly closed. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

Contraindications

Do not take Vemlidy if you are allergic to its active substance Tenofovir Alafenamide or any of the drug’s excipients.

Do not take the medicine if you have some of the rare hereditary problems such as galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption.

Do not use the medication if you suffer from serious kidney problems like end-stage kidney disease (a condition when the kidneys no longer work).

Do not take the tablets if you have problems with your bones as the drug may decrease bone density.

Vemlidy should not be administered with medications containing Tenofovir Alafenamide, Tenofovir Disoproxil Fumarate or Adefovir Dipivoxil, with some anticonvulsants (like carbamazepine, oxcarbazepine, phenobarbital and phenytoin), antimycobacterial drugs (such as rifampicin, rifabutin, and rifapentine), antivirals to treat HIV (e.g., atazanavir, darunavir, ritonavir, lopinavir), some antifungal medicines (e.g., itraconazole and ketoconazole), herbals (St. John’s wort).

Do not take Vemlidy during pregnancy or when breastfeeding without a doctor’s recommendation.

Do not give this medication to patients under 12 years of age without consulting a doctor.
